The CLF HelpLine is a free, personalized service supporting San Francisco 49ers Alumni navigating the outcomes of brain injury.

The Concussion Legacy Foundation (CLF) has partnered with the Golden Heart Fund to support the organization’s efforts helping former Niners navigate life after football by offering free brain health resources to former players and families.

CLF provides connections to vetted medical and mental health professionals with the expertise needed to quarterback your care. CLF also offers online support groups, 1-on-1 Peer Support Connections, and educational resources. Patients, spouses, and caregivers can access support through the confidential CLF HelpLine form linked below.

If you or a loved one are concerned about suspected CTE, are struggling with lingering symptoms, or suffered a recent concussion, we want to hear from you.

Collaboration

The Concussion Legacy Foundation is a nonprofit organization founded in 2007 to support athletes, veterans, and all affected by concussions and CTE. The Golden Heart Fund was established in 2017 to support 49ers Alumni during times of hardship.

This partnership honors the memory of Pro Football Hall of Famers Fred Dean, John Henry Johnson, and Joe Perry, as well as Dwight Clark, Greg Clark, and Larry Roberts. All were 49ers Alumni who battled various symptoms of neurodegenerative disease in their final years.

Their families shared their diagnoses publicly to raise awareness for the link between pro football and CTE, ALS, and similar diseases, knowing many other players faced similar challenges after football. CLF honors these players and their families by supporting the Golden Heart Fund’s passionate work connecting families with the help they need.
